Non-Clinical Cancer Social Prescriber — Community Navigator

Cardiff and Vale University Health Board is seeking a Macmillan Cancer Social Prescriber (Band 4) to support the Improving my Cancer Journey (ICJ) service. This role focuses on helping individuals affected by cancer through a non-clinical, community-based approach, including information and signposting to various services.

The candidate will work collaboratively with primary care teams and community organizations, ensuring personalized support is delivered to those in need. The postholder may work in both Welsh and English, pending language skills and community needs.
