1 x permanent contract – 28.5 hours part time (salary pro-rata)
The Autism and Learning Disability Team works borough wide with young people from the age of 14 transitioning into Adult Services. Part of the Adult Practitioner role will be to work closely with Children’s Services and SEND to ensure a timely and smooth transition.
You will also be expected to work in a person‑centred way to enable adults who are Autistic and/or diagnosed with a Learning Disability to reach their full potential, maximise their social inclusion and achieve positive outcomes.
The fixed‑term role is to complete the duty function for the team, screening all duty enquiries with support from the management team as required.
The permanent role is to work within our community hubs.
We use a strengths and asset‑based approach in our practice, supporting individuals through their strengths, the network of support from family, friends and local communities, ensuring care and support reflect assessed needs and strengths while promoting independence, choice and control.
