Clinical Support Worker

The Frailty team in Wirral University Teaching Hospital (WUTH) comprises a multi-disciplinary team (MDT) involving consultant geriatricians, junior doctors, advanced nurse practitioners (ANPs), specialist nurses for older persons (SNOP), clinical support workers (CSWs), physiotherapists, occupational therapists, integrated discharge team (IDT), social workers and third‑party representation from Age UK.
The clinical support workers are an integral part of the frailty service within Wirral University Teaching Hospitals and are often the first members of the team that patients meet alongside the SNOPs when attending the Emergency Department (ED), the Older Person’s Assessment Unit (OPAU) or the Urgent Medical Assessment Unit (UMAC).
The aim of the Frailty Team, as a whole, is to provide a rapid, holistic assessment of frail patients attending Wirral University Hospital and to formulate a management plan for their care adopting a “Home First” approach.
The role supports the SNOP by helping to facilitate early discharge from hospital and prevention of admission where it has been identified that patients can be managed in community settings.
The SNOP team currently comprises 6 Band 6 nurses, 2 Band 6 physiotherapists, 2 Band 6 occupational therapists and 3 Band 3 CSWs.
Main duties of the job
- Assist patients with hygiene, toilet and protection needs using personal initiative under the guidance of a team leader.
- Assist patients in promoting independence.
- Use a hoist to lift heavy patients and transfer them from bed to chair or wheelchair.
- Assist patients onto a commode.
- Access patient condition and record basic observations of temperature, pulse, blood pressure and respiration rate.
- Use judgement to inform senior staff of abnormal findings.
- Assist in the care of dying patients, including providing support to relatives.
- Provide support for relatives.
- Assist with last offices.
- Assist with patient transfer to CT scanner, MRI and theatre.
- Assist with patient transfer for intra‑ and inter‑discharges from the hospital.
- Be able to prepare equipment in an emergency.
- Maintain effective communication with patients and staff.
- Communicate effectively with agitated and confused patients, including those with physical and mental disabilities.
- Be aware of and maintain patient confidentiality requirements in the clinical setting.

Person Specification
Qualifications, Specific Experience & Training
- A good standard of education to A‑Level standard and the agreement to undertake the relevant Level 3 qualification on appointment.
- Experience working in a clinical environment and undertaking a range of relevant clinical skills with proven competencies.
- Experience working in the NHS.
Knowledge and Skills
- Evidence of competence in relevant specialist skills or tasks.
- Knowledge and understanding of the responsibilities of a Clinical Support Worker.
- Previous experience and training in manual handling patients using appropriate equipment.
- Understanding of the importance of promoting patient independence.
- Ability to provide support and reassurance to patients.
Personal Attributes
- Ability to work well as part of a multi‑disciplinary team.
- A positive attitude and ability to act on feedback.
- Polite, friendly, approachable and self‑motivated.
