Adult’s Social Worker – Discharge to Assess Team
We are looking for an Adult’s Social Worker to join a Discharge to Assess Team.
This role requires a Social Work Qualification with a minimum of 2 years permanent experience within a local authority.
About the Team
The team supports adults following hospital discharge, completing assessments and reviews to ensure safe and effective support plans are in place. Duties include Care Act assessments, support planning, BI assessments, Mental Capacity assessments, safeguarding enquiries and duty cover responsibilities.
The service also works closely with funding panels and multidisciplinary professionals to support positive outcomes for service users.
About You
Experience working within Adult Social Care is essential for this role.
Strong knowledge of the Care Act, Mental Capacity Act and safeguarding legislation is required.
Experience completing assessments, reviews and support planning within a fast‑paced service is also essential.
A degree within Social Work (DipSW/CQSW) with a minimum of 3 years post‑qualified experience is essential for this post.
