Specialist Therapist (Frailty)

We are looking for a full‑time Band 7 Therapist specialising in frailty to join the Frailty Service at Rochdale Infirmary, supporting residents in Heywood, Middleton and Rochdale (HMR). You will work with doctors, advanced practitioners, nurses, therapists, and pharmacists to support frail patients in hospital and in the community, promoting a home‑first approach.

Key Responsibilities

  • Deliver the frailty service, ensuring timely and appropriate care is provided in patients’ own homes.
  • Work closely with patients and carers to manage health needs, promote self‑care and develop personalized care plans.
  • Collaborate with multidisciplinary teams to shape high‑quality, patient‑centred services aligned with Trust goals.
  • Take a leadership role within the team, supporting and inspiring colleagues in a fast‑paced setting.

Qualifications & Experience

  • Registration with the appropriate professional body (e.g., NMC/HCPC).
  • Degree qualification or equivalent.
  • Recognised teaching or mentor qualification.
  • Previous experience of team leadership.
  • APLS/ALS certification.
  • Level 7 clinical skills.
  • V300 (non‑medical prescribing) or willingness to undertake.
  • Extensive knowledge and experience in urgent and/or community setting.
  • Knowledge and understanding of local health and social care pathways.
  • Understanding of long‑term conditions including frailty and end‑of‑life processes.
  • Evidence of planning and leading service development; involvement in policy and protocol development.
  • Ability to travel across the Trust footprint as required.
  • Full UK driving licence and willingness to use personal vehicle for work.
