Job Overview
To provide day‑to‑day care for service users under the supervision of a Clinician which adheres to best practice, standards and guidelines, and contributes towards the service users care and treatment.
- Work in partnership with service users, other members of the MDT staff as appropriate, to achieve the optimum health outcome for clients.
- Be part of the MDT in providing a high and consistent level of holistic care, maintaining the dignity and equality of the individual. This includes direct care.

Your work will involve working with elderly and vulnerable people who have mental health problems such as dementia and depression. Your role will include enabling people to stay living as independently as possible, to take part in activities in their local community and supporting them to maintain good mental and physical wellbeing.

Experience
- Essential criteria: Significant experience of working with people with dementia.

- Worked in a mental health care setting.

- Evidence of working as part of the MDT.
- Evidence of good communication skills.
- Evidence of working autonomously.
- Desirable criteria:
- Ability to speak Welsh.
- Evidence of IT skills.
