Our service operates from 5pm to 8am Monday–Sunday.

A full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Applicants will be required to pay for their DBS check. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced). This cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service, which carries an annual fee of £16.

Main duties of the job

We are looking for Community Healthcare Support Workers who are committed and passionate about promoting health, well‑being and independence. They must be a dedicated and enthusiastic clinician who believes in providing patient‑centred care.

A key aim of our service is to optimise patient care within their own homes and reduce unplanned admissions.

Collaboration is key. We work closely with colleagues across the region to provide a holistic, multidisciplinary approach to care delivery.

Detailed job description and main responsibilities

You should possess good interpersonal and time management skills, be a good communicator, organised and able to prioritise and work well as a member of a team. You will support community nurses when required in patients’ homes and demonstrate a professional manner at all times to patients, relatives and staff of all levels.

Part‑time hours are available between 24 and 36 hours per week. Evening shifts are 5pm–11pm and night shifts are 7pm–8am. You will be required to work a combination of both shift patterns and cross over into the South Nottinghamshire areas when required.

We do not meet the pay level required for a Skilled Worker visa; therefore the Trust is not able to sponsor for a Skilled Worker / Health & Care visa. Applicants requiring a Skilled Worker Visa can determine the likelihood of obtaining a Certificate of Sponsorship against the relevant criteria.
