Community Nurse – Sudbury Integrated Neighbourhood Team

An exciting opportunity for Community Nurses has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way.

Key Responsibilities

  • Provide nursing services to patients in the community.
  • Carry out home visits and assess patients, planning and implementing an individualised care plan.
  • Deliver care to virtual ward patients.
  • Use treatment facilities at the health centre, including specialist clinics.
  • Work independently with support from the multidisciplinary team within the hospital and across the West Suffolk Alliance.
  • Provide safe, effective treatments in a community setting, including palliative care, wound management and catheter management.

Qualifications & Requirements

  • Thorough understanding of community nursing.
  • Confidence in assessment skills and ability to provide safe, effective treatment.
  • Experience of palliative care, wound management and catheter management preferred.
  • Car ownership and driving licence.
  • Availability to work a 7‑day service within core hours 8 am to 6 pm.

Working arrangements

The post is part‑time with the possibility of full‑time as an option.

Additional information

This role is suitable for newly qualified and experienced nurses. Training and education will be available to support career progression.
