Overview
A 12‑month Fellowship based at Harefield (HH) and Guys Hospitals (GH) joining the wider GSTT Department of Infection. The post offers the opportunity to manage infections in complex non‑HIV immunocompromised patients—heart, lung and renal transplant recipients, those with haematological or solid organ malignancy, patients on systemic anti‑cancer therapy, and those on renal dialysis—as well as experience in managing complex cardiothoracic infections including peri‑myocarditis, endocarditis, empyema, and device‑related infections (cardiac and orthopaedic).
The Fellowship is divided into six months at each site (GH and HH). Postholders will work closely with site‑based Infection Consultants, other doctors in training, Infection Control staff, and Infection Pharmacists.
Main duties
- Transplant infectious diseases—heart and lung at HH; renal and pancreas at GH.
- Non‑HIV immunocompromised patients—systemic anti‑cancer therapy, haematological and solid organ malignancies, CAR‑T therapy, and routine chemo/immunotherapies.
- Cardiothoracic infections—empyemas, peri‑myocarditis, endocarditis, complex cardiac device‑related infections, mechanical circulatory support infection issues and post‑surgical infections.
- Infections in patients with renal failure and urological problems—dialysis‑related infections and infections associated with stone disease, prostatitis, epididymo‑orchitis, and post‑surgical infections.
- Orthopaedic infections—native bone and joint, metalwork‑associated infections, and spinal infections.
- ENT and head & neck infections—NecOEs and base of skull osteomyelitis, retropharyngeal and mediastinal infection.
- Acquisition of laboratory liaison skills in Microbiology and Virology, applied to ward‑based consult services at Guy’s, St. Thomas’, Royal Brompton or Harefield sites.
Clinical work and teamwork
Most clinical work involves liaison with diverse clinical specialties on each site, including bedside assessments, microbiological or virological result reviews, and telephone advice. Both GH and HH have multiple consult rounds and multidisciplinary team attendances. The Infection Service operates as a consult service with 2 consultants at HH and 3 at GH.
GH attachment: 6‑month period split into two 3‑month blocks; each block works alongside an ID consultant supporting Haematology, Oncology, ICU, Orthopaedics, ENT (block 1) and Renal, Transplant, Urology, Thoracic (block 2). Activities include specialist MDTs, weekly ward rounds, daily ward consult reviews, weekly stewardship rounds and IPC liaison.
HH attachment: 6‑month period focusing on adult ICU, cardiac and thoracic surgical, endocarditis (complex, tertiary referral) and transplant ID MDTs. Activities include daily transplant meetings with critical care and surgery, a weekly transplant ID round, a monthly fungal MDT, and opportunities to work with the national transplant ID network.
Hours and on‑call
The hours of work are 09:00‑17:30 Monday to Friday, with agreed on‑call duties. On‑call responsibilities are part of the New Deal on Doctors hours, European Working Time Directive and New Junior Doctors Contract. GH on‑call includes a 1‑in‑9 weekday night rotation for Infection (Microbiology and Virology) and High Consequence Infectious Diseases coverage when activated. HH on‑call follows the RBHH rotation with 1‑in‑6 split weekends and evening on‑calls, potentially done remotely.
Person Specification – Qualifications and Training
- MBBS or equivalent.
- Full GMC Registration with licence to practise.
- Full MRCP (UK) or equivalent.
- Higher degree (e.g. MSc, PhD, MD).
Clinical Experience
- Experience managing infections across a range of patient groups.
- Registrar‑level experience working in Clinical Microbiology, Virology or Clinical Infectious Diseases departments.
- FRCPath Part 1 or CICE examination.
Knowledge and Skills
- Effective member of a multidisciplinary team.
- Prioritising clinical need.
- Working independently where appropriate.
- Proven ability to conduct clinical audit.
Research
- Understanding of clinical research principles and applications.
- Evidence of original research.
Teaching
- Experience of undergraduate and postgraduate training.
Management
- Understanding of clinical management roles.
Other
- Understanding and adherence to Good Medical Practice set out by the General Medical Council.
