Healthcare Assistant (HCA) at Clayton Brook Surgery, working 24 hours per week (ideally Monday, Wednesday & Friday). In addition to this, the successful candidate will also provide 13.5 hours per week supporting our medical coordinator (Med Co) work, bringing the role to a full‑time position.

The successful candidate will occasionally be required to work at our partner practice, Moss Side Medical Centre in Leyland, and therefore mobility and flexibility across sites is essential.

The successful candidate will play a key role in supporting our clinical team to deliver high‑quality, patient‑centred care. You will assist with health checks, phlebotomy, ECGs, long‑term condition monitoring, vaccination support, and health promotion activities.

Experience working within General Practice is essential, and confidence using EMIS Web would be an advantage. This role offers the opportunity to contribute directly to improving patient outcomes, supporting our QOF and Long‑Term Conditions (LTC) LES targets, and making a real difference within the local community.

Main duties of the job

— Undertaking routine observations such as blood pressure, pulse, temperature, height, weight and BMI;

— Performing phlebotomy and ECGs; conducting urinalysis; and assisting in long‑term condition (LTC) reviews including frailty, respiratory, cardiovascular, diabetes and hypertension;

— Recording QOF indicators such as smoking status, alcohol intake, BMI and blood pressure; assisting with flu and B12 injections; carrying out NHS Health Checks, frailty assessments and lifestyle screening;

— Providing basic wound care (if trained), delivering health promotion advice and encouraging patients to engage in self‑care and prevention;

— Ensuring accurate clinical coding in EMIS Web; maintaining infection control and CQC standards; supporting QOF/LES achievement through accurate documentation; and embracing teamwork, flexibility and continuous learning through meetings, audits and mandatory training.

  • Assisting with the collection and collation of data on needs related to health and well‑being
  • Administering influenza, shingles, pneumococcal, RSV and B12 injection under the supervision of the Practice Nurse in accordance with practice protocol and patient specific directive
  • Covering holidays and sickness at short notice and being as flexible as possible
  • Participating in audits where appropriate

In the performance of the duties the post‑holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

The post‑holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the Practice Health & Safety Policy, to include: using personal security systems within the workplace according to Practice guidelines; identifying the risks involved in work activities and undertaking such activities in a way that manages those risks; making effective use of training to update knowledge and skills; using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards; and reporting potential risks identified.

Qualifications

  • Experience working within General Practice
  • Use of EMIS Web
  • Good understanding of quality and performance frameworks such as QOF, Long‑Term Conditions (LTC) LES, and other locally commissioned Enhanced Services.
  • GCSE grade A to C in English and Maths
  • Level 2 Diploma in Health and Social Care (or equivalent)
  • Care Certificate: Skills for Care, Skills for Health and Health Education England
  • Demonstrable competency in Chronic Disease Reviews
  • Qualified in Phlebotomy
  • Qualified to administer flu, B12, RSV, shingles and other vaccinations.
