Job Summary

Connected Health requires a proactive and dynamic individual to join our Growth and Expansion Team as a Rapid Responder. The role supports the Registered Manager in developing new home care services within the allocated Trust zone.

Key Responsibilities

  • Work exclusively with the Registered Manager as part of the Expansion Team
  • Support development and mobilisation of new home care services
  • Provide cover and support across the allocated Trust zone
  • Assist with new care packages, hospital discharges and emergency cover
  • Support staff shadowing and ensure Health and Safety policies are followed
  • Check and collect daily record sheets from clients’ homes
  • Deliver care plans and log sheets to clients’ homes when required
  • Deliver PPE and essential documentation or supplies
  • Support the care team to ensure services are delivered safely, consistently and to a high standard
  • Travel between clients’ homes, offices and service areas within the Trust zone
  • Undertake additional duties as required

Benefits

  • £13.60 per hour
  • 40 hours per week
  • Block pay
  • Alternate weekends

Essential Criteria

  • Minimum of 6 months previous homecare experience
  • Excellent verbal and written communication skills in English
  • Full clean driving licence
  • Access to a car for work purposes
  • Ability and willingness to travel throughout the Trust zone
  • Strong organisational and time management skills
  • Flexible approach to working hours and service needs
  • Ability to work closely with the Registered Manager to support new and developing services

Desirable Criteria

  • Previous supervisory or senior care experience
  • Excellent interpersonal skills
  • Empathetic and professional approach to supporting clients and staff
  • Ability to work independently and as part of a team
  • Confidence in responding to changing operational priorities
  • Experience supporting service growth, new care packages or hospital discharge pathways
