About the Role

Support a female with Epilepsy and Complex Learning Disabilities and deliver AFG’s vision of “a world where people control their lives” in line with each individual’s personalised plan.

Responsibilities

  • Provide all health‑related and social support to people with diagnoses such as Learning Disability, Autism and Mental Health.
  • Support people who present with behaviours that challenge by using appropriate techniques and strategies.
  • Provide safe medication and pain management in line with medication care plans.
  • Manage medical equipment and supplies, monitor depletion and re‑order in a timely manner to avoid shortages.
  • Attend all clinical mandatory and specialist training sessions, face‑to‑face and online, to remain competent in care delivery.
  • Report and record all incidents that cause or have the potential to cause harm in a timely manner, in line with AFG policies and procedures.
  • Support people to design, develop and own the support they receive, contributing effectively to Personalised Planning.
  • Support people to make informed choices and increase their independence, following the principles of the Mental Capacity Act when required.
  • Support people to exercise their cultural beliefs and practices.
  • Enable people to form and maintain relationships of their choice.
  • Support people to maintain their home/living environment.
  • Promote and increase independence for the people we support.
  • Support people to access their community.
  • Enable people to take positive risks in their lives.
  • Ensure the people we support are not abused, neglected or ill‑treated.
  • Address any poor treatment, make the situation safe and report concerns to line management, the On‑Call Manager or via the Whistleblowing procedures.

Enabling Support

  • Provide personalised support to people with complex care needs, including assistance with activities of daily living, medication administration and managing medical equipment and supplies.
  • Actively participate in helping the people we support to achieve their outcomes as per their agreed Personalised Plan, providing safe and effective care.
  • Support people to develop and maintain relationships, as identified in the individual’s Personalised Plan, and facilitate new relationships.
  • Support people to make informed choices around a healthy lifestyle.
  • Support people to meet their leisure, work, social, emotional and spiritual needs.
  • Support people to connect with their community in accordance with their wishes as identified in their Personalised Plan.

Training & Development

  • Participate fully in training and development in accordance with the Alternative Futures Group development programme and any external training as required.
  • Attend and actively participate in supervision sessions and performance reviews.
  • Attend and actively participate in team meetings.
  • Develop and share skills to aid in the provision and improvement of support.

Person Specification – Essential Criteria

  • Ability to support people in ways consistent with their rights, choices and responsibilities.
  • Positive attitude towards people with complex physical, learning disabilities and/or mental health problems.
  • Ability to provide sympathetic, emotional and practical support to others.
  • Ability to support others to make informed choices within their daily lives.
  • Good verbal communication skills and ability to listen sensitively to others.
  • Written English skills, with an ability to contribute accurately to record‑keeping.
  • Good interpersonal skills.

Experience

  • Minimum of 2 years experience in a Health & Social Care paid or unpaid role.

Personal Attributes

  • Willingness to attend all clinical mandatory and specialist training courses and events.
  • Ability and willingness to work flexible hours, covering 24 hours/7 days.

Working Environment

Support Workers work in a variety of settings, including community-based services, Independent Hospitals, or Adult Care Homes.
