Mental Health Practitioner

We are pleased to announce two Mental Health Practitioner posts to join our St Helens Recovery Team.

Key Responsibilities

• Deliver high quality care and hold a caseload of service users, collaborating with other disciplines to provide comprehensive assessments, advice, and treatment.
• Coordinate the care of service users, carrying out assessment planning, implementation, and evaluation under the Care Programme Approach and line manager supervision.
• Develop care plans that meet the assessed needs of individuals, ensuring that unqualified staff do not deliver care as identified in the plan.
• Liaise with family members, carers, and significant others during assessments and care planning.
• Undertake and supervise risk assessments, formulating risk management plans for service users under your care.
• Ensure Safeguarding Children and Adults requirements are followed and participate in relevant meetings.
• Work collaboratively with other services, external agencies, families, and carers to identify personalised goals and tailored support for recovery.
