This is a dynamic, community-focused position within a centralised model of care. You’ll design and deliver monthly themed health initiatives, offer 1:1 support, and lead group sessions aimed at improving wellbeing among groups such as older adults or frequent attenders. Your work will directly address social determinants of health, connecting people to non-clinical support and empowering them to thrive.
You will work in partnership with the 7 GP practices and other care providers that are based in the central and north Derbyshire Dales from Matlock in the centre up to and including Hope village on the South Yorkshire Border.
Key Responsibilities
- Lead monthly, data-informed health and wellbeing campaigns
- Facilitate group sessions to support community resilience
- Manage a caseload with autonomy and purpose
- Work closely with GPs, voluntary sector partners, and local services
- Contribute to weekend community events and outreach
- Use clinical systems (e.g., EMIS, SystmOne) to track impact and outcomes
- Engage in peer support, supervision, and continued professional development
- Lead proactive, population-based thematic interventions informed by population health data and practice insights
- Build strong links with local GPs, community services, and voluntary sector organisations to strengthen referral pathways and support
- Participate in weekend community events, engagement initiatives, or group activities as required
- Record activity, track outcomes, and support service evaluation using clinical systems and PHM tools
- Engage in monthly clinical supervision and peer learning opportunities, sharing best practices with local and regional networks
What We Are Looking For
- Experience in community-facing, health, or social care roles
- Proven ability to work independently and manage a caseload
- Experience facilitating or coordinating group sessions or wellbeing initiatives
- Strong communication, organisational, and interpersonal skills
- Awareness of safeguarding, confidentiality, and professional boundaries
- Digital literacy and familiarity with clinical or data systems
- Training in motivational interviewing, coaching, or trauma-informed care
- Familiarity with social prescribing within PCNs or Integrated Care Systems
- Experience working with at‑risk or underserved populations
- Full driver’s licence and access to a vehicle (mileage paid)
- Completed NHS England online learning programme via the Personalised Care Institute website or equivalent Level 3 occupational standard qualification
- Engagement in peer support networks and regional Integrated Care System (ICS) activities
