Band 3 Assistant Patient Pathway Coordinator (CAT 9 Diabetes and Endocrinology)
This is a part-time post for 30 hours per week.
An exciting opportunity has arisen for a motivated individual to join our Diabetes and Endocrinology Team to assist the Patient Pathway Manager and Patient Pathway Coordinators by carrying out delegated responsibilities for a range of operational activities, working autonomously using initiative and their specialist knowledge and experience of the service to make decision.

Responsibilities
- Specialise in specific modality appointment bookings, but have an overview and knowledge of all booking processes.
- Ensure full compliance with scheduling guidelines, protocols and clinical targets whilst identifying and resolving breach situations and the consequences of changes to timetables and potential impact on capacity.
- Be the point of contact for all administrative issues relating to patients’ pathway of care.
- Work as part of a multidisciplinary team and provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
- Ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.
- Provide a consistent approach across the Trust; Standard Operating Procedures (SOPs) will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered.
