About the Team
The Learning Disability and Autism team ensures that every person with a learning disability or autism has access to support, services and opportunities. The team works with adults across Rotherham, from transitions services to older adults who require reviews of their needs. Support is holistic, working with professionals, families and carers.
Responsibilities
- Conduct assessments under the Care Act 2014 and the Mental Capacity Act, and conduct Best Interests and Section 42 safeguarding enquiries.
- Co-create tailored care and support plans that empower autonomy and independence.
- Collaborate with families, carers, health, housing and voluntary sector partners to deliver holistic, integrated support.
- Monitor and review plans regularly to ensure they evolve with the individual’s changing needs.
- Advocate for service users, championing their rights and choices.
- Maintain clear records and prepare professional reports for case conferences and reviews.
About You
- Qualified (BA/MA/DIPSW) with valid Social Work England registration.
- Proven experience in adult social work, particularly supporting autistic adults and those with learning disabilities.
- Excellent knowledge of relevant legislation, including Care Act, Mental Capacity Act, Mental Health Act and safeguarding frameworks.
- Strong assessment, analytical, planning and risk management skills.
- Confident communicator with ability to build relationships across agencies.
- IT literacy and case management system experience.
