Responsibilities

• Assess patients’ social care needs and create tailored plans to facilitate effective hospital discharge and ongoing community support.
• Collaborate with multidisciplinary teams to ensure a seamless transition of care for patients leaving the hospital.
• Provide emotional support and practical advice to patients and their families, helping them to access necessary services and resources.
• Document and record all necessary information accurately to maintain high standards of patient care.
• Liaise with external agencies and community partners to maximise the support available for discharged patients.
• Regularly review and evaluate care plans to ensure they continue to meet patients’ needs effectively.
