We are seeking a dedicated and compassionate Neighborhood Support Team Worker to join our Adult Social Care team in East Sussex. This is a rewarding opportunity to make a meaningful difference in the lives of vulnerable adults within the community, supporting independence, wellbeing, and safety.

Key Responsibilities

• Provide high-quality, person-centred support to adults with a range of needs
• Work collaboratively with multidisciplinary teams to assess, plan, and deliver care
• Support individuals in maintaining independence within their own homes and communities
• Conduct risk assessments and contribute to safeguarding processes
• Maintain accurate and up-to-date case records in line with professional standards
• Engage with families, carers, and external agencies to ensure holistic support

Requirements

• Experience in Adult Social Care or a similar community-based role
• Strong understanding of safeguarding and relevant legislation
• Excellent communication and interpersonal skills
• Ability to manage a caseload effectively and work independently
• Relevant qualification in health and social care (desirable)
