Job Overview
We are looking for a registered nurse to work as part of a multidisciplinary team that identifies and case-manages patients with complex discharge needs. You will operate across the Lincoln County Hospital site, assessing patients and ensuring timely discharge planning. You will work with patients, families and ward‑based staff to implement a strength‑based approach, lead MDT meetings and support discharge coordinators.
Main duties of the job
- Plan and coordinate discharge for patients with complex needs.
- Advocate for patients and carers, ensuring individual needs are met.
- Ensure safe discharge of all patients.
- Lead and support multidisciplinary team (MDT) meetings.
- Collate and present discharge summaries to relevant stakeholders.
Person specification
Qualifications
- Active professional registration.
- Degree level or equivalent.
- Evidence of continuous professional development and post‑registration coursework.
- Evidence of supervisory experience.
- Demonstrated ability to deliver evidence‑based, high‑quality care.
- Completion of preceptorship.
- Previous experience with discharge planning and MDT working.
- Excellent written, verbal and IT communication skills.
- Strong understanding of safeguarding and good practice.
- Teamworking and independent working skills with initiative.
- Strong prioritisation and workload management.
- Commitment to ongoing personal and professional development.
