We have an exciting opportunity for a Community Support Worker within Home First South Kerrier Team. The main purpose for this post is to provide a versatile and responsive service to ensure opportunities for patients’ reablement are optimised within the community setting. The Reablement workers within each locality will form part of an Integrated Care Pathway for older patients involving a range of services to prevent hospital admission and support discharge from hospital.

Main duties of the job

  • Working as part of an interdisciplinary team to facilitate holistic reablement for patients.
  • Work under the guidance and clinical supervision of the locality team including ward and community staff.
  • Provide an 8‑8, 7‑day service for the reablement of patients.
  • Provide, receive and give feedback on routine information requiring tact and persuasive skills.
  • Persuade patients to cooperate in their reablement programme; require tact and reassurance when dealing with patients.
  • Begin working towards the completion of work‑based Rehabilitation Support Worker and Healthcare Assistant (HCA) qualifications.
  • Complete the Ageing Well competencies training programme.
  • Actively participate in own wider learning, including clinical supervision and peer supervision.
  • Work alongside a clinical facilitator to achieve venepuncture competency, as required.
  • Work within parameters of own job description, monitor own progress; strive to improve practices and performance as services evolve.
  • Assist the patient to achieve their reablement goals using a range of specific activities, primarily on an individual basis, but also in groups, without direct supervision. This may include activities for developing independence in daily living skills, such as supporting with personal care and promoting a return to normal function and activity: support and encourage the appropriate use of aids/equipment and reinforce self‑care wherever possible.

Person specification

Education / Qualifications and Relevant Experience

  • NVQ 3 or equivalent experience in rehabilitation, health and social care.
  • Previous experience of providing care/reablement in a health or social‑care setting.
  • An awareness of the needs and aspirations of those requiring assistance.
  • Food hygiene certificate.
  • Knowledge of infection control.

Skills and Aptitude

  • Ability to work flexibly, independently and as a lone worker.
