Clinical Pharmacist – CYP Primary Care ADHD Prescribing Service

We are looking to recruit a Clinical Pharmacist who will work within the South West Herts Enhanced Primary Care Monitoringservice to provide support and intervention to children and young people withADHD (as well as co-morbidities) between the ages of 6 and 18.

The postholder will be confident in completing ADHD medication reviews utilizing specialist skills and using clinical interview techniques in order to ascertain a childs response to medication and manage potential sideeffects. They will be expected to be confident in ascertaining potential symptoms of co-morbidity such as ASD,mental health needs or physical health concerns and document information accurately and refer to appropriate local services.

You will be experienced in initiating and titrating ADHD medication in children and young adults and confident with the formulary.

Main duties of the job

This is a clinic-based role, and you will carry out holistic reviews of patients referred as appropriate by HPFT CYP ADHD service after diagnosis into this primary care monitoring service. Clinics are based within the 4 localities of South West Herts; Dacorum, St Albans, Hertsmere and Watford and 3 Rivers, this role will be based across 3 sites.

Apply knowledge of behavioural management strategies and evaluate responses to these and medication.

Ability to communicate orally, electronically and in writing with children, young people, and families.

Correspond with GP supervisors in the team, multi professional agencies, families, children services and voluntary/ statutory services. Attend weekly MDT meetings with senior team to discuss and review caseload regularly.

Knowledge of referral pathway for complex cases.

Work autonomously in clinic to provide holistic medication reviews to children and young adults in relation to the management of their ADHD medications and to adjust drug treatment appropriately with the support of senior team and working within agreed protocols and guidelines with the recommendation of continued prescribing shared to the child/ young persons GP within recommended guidelines of the NMC medicine management code of conduct and NICE guidance.

Provide an integrated service working closely with wider team and liaising with colleagues and clinical GP Lead.

As an autonomous practitioner, be responsible for monitoring patients with an ADHD diagnosis on medication through Clinical pharmacist-led clinics, communicating change to other members of the team and giving guidance on pharmaceutical treatment of Children and Young People up to the age of 18. The post-holder will be able to take a holistic approach to care, considering a variety of aspects which may impact on the child’s function and behaviour and ensuring to hear the child’s voice within their review process.

Contribute to the development of leaflets and information regarding medication and ADHD for children, families, and professionals.

Ensure the effective and efficient use of physical and financial resources.

Collect, collate, evaluate, and report information and maintaining accurate patient records.

Ensure the service is monitored on a regular basis and data is available as a record to provide statistical information to Clinical Leads and others and to provide data to relevant local and national bodies to audit service as appropriate.

  • GPhC Registration

Skills Abilities and Knowledge

  • Knowledge of CYP legislation and relevant mental health legislation.
  • Understanding of evidence-based practice and the ability to demonstrate how this influences clinical practice.
  • The ability to assess, formulate and manage care appropriately, ensuring discharge is timely.
  • To be able to function as a part of a multi-disciplinary team and to liaise effectively with other agencies i.e., education, GP’s, maintaining positive working relationships.
  • Ability to maintain effective clinical records both written and electronic.
  • Ability to manage and organise own time and diary.
  • An awareness of alternative and augmentative communication system
  • Having a sound understanding of theoretical frameworks that underpin the understanding of behaviour
  • Knowledge of children’s mental health and Child Development

Experience

  • Experience of working effectively with child & young people.
  • Experience of working with people with mental health difficulties.
  • Experience in managing the co-morbidities of ADHD and provision of Behavioural advice.
  • Experience in assessment and management of children with ADHD.
  • Safeguarding and Adult Level 3
  • Experience of developing and providing evidence based assessments, formulations, and care plans.
  • Experience of liaison and working with a range of other relevant agencies and professionals.
  • Experience of mentoring and supervision.
  • Understanding of the principles of Clinical Governance and a willingness to follow HertsOne Policy and Procedures
