We are looking for enthusiastic, experienced Advanced Care Practitioners to become part of our established and forward-thinking team, delivering dedicated support to care home residents and housebound patients across Bridlington.

At Drs Reddy & Nunn, we provide proactive, responsive healthcare to a population of around 42,000 patients, alongside specialist support for 24 care homes in the local area. Our highly regarded service is built on strong partnerships with care home providers, GPs, community services, and wider system colleagues – working together to deliver truly person-centred care for residents and patients with complex and often vulnerable health needs.

We are recruiting to a combined establishment of 1.5 Whole Time Equivalent (WTE) and a keen to hear from applicants seeking both full-time and part-time opportunities, offering flexibility to suit your work-life balance.

Main duties of the job

As an Advanced Care Practitioner, you will work autonomously and as part of a multidisciplinary team, providing advanced clinical assessment, diagnosis, treatment and care planning for care home residents and housebound patients with complex physical, mental health and social care needs.

The successful post holder will primarily support care homes residents and housebound patients, whilst also providing clinical support to the wider practice team through clinics and service delivery as required to meet patient demand and organisational priorities.

You will act as a key clinical link between primary care, care homes, community services and wider health and social care partners, supporting proactive management of frailty, long-term conditions and urgent healthcare needs.

In addition to your care home and housebound patient responsibilities, you will provide clinical support to the practice through face-to-face, telephone and digital consultations, long-term condition management, urgent care assessment and other clinical clinics as required, helping to ensure safe, effective and responsive patient care across the practice population.

Significant post-registration clinical experience.

Experience of working autonomously at an advanced clinical practice level.

Experience of advanced clinical assessment, diagnosis and clinical decision-making.

Experience

  • Experience working within care homes.
  • Experience working in primary care, community services, urgent care or frailty services.
  • Experience of Enhanced Health in Care Homes (EHCH).
  • Experience of service development, quality improvement or transformational projects.
  • Knowledge of frailty, multimorbidity and complex care management.
  • Understanding of Comprehensive Geriatric Assessment (CGA).
  • Knowledge of safeguarding adults and vulnerable people.
  • Knowledge of the Mental Capacity Act and Best Interest decision-making.
  • Understanding of clinical governance, patient safety and quality improvement.
  • Understanding of evidence-based practice and professional accountability.
  • Knowledge of advance care planning and end-of-life care principles.
  • Advanced clinical assessment and diagnostic skills.

Qualifications

  • Registered Healthcare Professional with the NMC, HCPC or GPhC.
  • Master’s Degree in Advanced Clinical Practice (or equivalent recognised ACP qualification) or currently working towards completion.
  • Evidence of ongoing Continuing Professional Development (CPD).
  • Independent Prescribing Qualification or working towards completion.
  • Advanced Clinical Assessment qualification.
  • Qualification in Frailty, Care of Older People, Palliative Care or Long-Term Conditions.
  • Teaching, mentorship or clinical supervision qualification.
