It is an exciting time to join the Haemoglobinopathy team at Sandwell and West Birmingham NHS Trust as we look forward to a new working model of integrating community and hospital Sickle Cell and Thalassaemia Service. The team is looking for a friendly and dynamic nurse to join in a liaison nurse role, an integral part of facilitating the transition between paediatric services, and increasingly bridging the gap between hospital and home.
We are looking for someone with experience of caring for people with haemoglobinopathy conditions or awareness of the challenges they face. You will need to have excellent communication skills, and be competent in venepuncture, cannulation and the use of CVADs. Teaching, counselling or prescribing qualifications are desirable but a willingness to develop is essential.
Main duties of the job
All aspects of care of the haemoglobinopathy patient: screening and counselling on diagnosis.
Clinically supporting patients with their well being in an outpatient setting but also supporting during inpatient stays, delivering day treatments and managing the unwell patient.
Coordinating and participating in regional MDTs, teaching and raising awareness throughout hospital and community.
This role involves visiting patients in their own homes to deliver a holistic approach to care. As it requires working within the community, a full UK driving licence and access to a personal vehicle are essential.

Experience
- Post registration experience in acute setting
- Involvement in teaching and learning initiatives
- Evidence of supporting others in a clinical environment
- Experience of audit initiatives and other quality initiatives
- Able to demonstrate commitment to high quality patient care
- Experience and evidence in implementing and managing change
- Evidence of implementing research based practice
- Care of patients with chronic illnesses
- Experience of liaison or community work
- Nurse prescriber or willingness to undertake should the role demand this
Qualifications
- ENB 998 (teaching and assessing) SLAIP, D32 or D33 or equivalent course
- Venepuncture and Central Venous access device competence
- Haemoglobinopathy course
- Leadership course
