Nursing Associate: Home Visits & Geriatric Care

NHS is looking for a Nursing Associate to join the Tonbridge Primary Care Network. This role involves managing and supporting patients with frailty, and conducting Comprehensive Geriatric Assessments (CGAs) as part of a multidisciplinary team.
