Overview

We have an exciting opportunity within our Acute Inpatient Older Adult Dementia Service for a Senior Acute Care Nurse to join our team. We are looking for an experienced and passionate clinician who is self-motivated, compassionate and willing to work as part of a team, with a can-do attitude to be successful in this role.

Langworth Ward is an 18 bedded acute assessment and treatment ward for older adults experiencing mental health problems associated with Dementia (we do take younger individuals depending on diagnosis). The ward is challenging and fast-paced, offering excellent opportunities for professional development.

Responsibilities

  • Coordinate the shift and support the delivery of excellent nursing care as a Senior Acute Care Nurse.
  • Lead and manage a team of staff nurses and healthcare assistants when on shift.
  • Support the Ward Manager with operational duties and help develop and maintain clinical standards.
  • Risk assess and manage complex and challenging behaviours of acutely unwell patients and implement reducing restrictive practice.
  • Ensure and undertake the assessment, planning, evaluation and review of individualised plans of care in conjunction with the patient.
  • Complete assessments of needs and deliver individualised care to support a successful transition and discharge to the appropriate environment when recovery is optimised.
  • Be visible and accessible to patients, families and carers, working together to ensure efficient, effective and caring services.
  • Demonstrate sound clinical skills and the ability to formulate collaborative care plans.
  • Maintain a high standard of written and verbal communication.
  • Work alongside service users and carers/relatives to continually improve and maintain high standards.

Qualifications and requirements

  • Sound clinical skills with the ability to formulate collaborative care plans.
  • Ability to work flexible shift patterns across 24 hours a day, 7 days a week in a busy inpatient environment.
  • Emotional resilience and the capacity to support service development and enhancement of services.
  • Strong written and verbal communication skills.
