The role offers a structured schedule working Monday to Friday, 9 AM to 5 PM, providing early assessment, evidence‑based interventions, and fostering strong community support links for individuals with serious mental illness within North and South Tandridge Primary Care Networks (PCNs).

Responsibilities

• Engage with community resources to enhance support networks for patients.
