Salary:31,537 – 33,699 per annum

In this rewarding role, youwill manage your own caseload and work closely with customers, carers, and partner agencies to deliver person-centredcare.Working collaboratively with Occupational Therapists, Social Workers, and multi-professional teams, you will promote independence, choice, and wellbeing using universal services and community resources.

As thisis a fast-paced team, you will lean on your organisational abilityto manage caseloads effectively and have excellent communication skills to be able to work collaboratively.In addition to a sound understanding of the Care Act 2014, you will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.

You will be providing outcome-focused assessments toidentifythe right support for positive life changes and creating and reviewing support plans that make a real difference.
