We are looking for experienced Community Health & Social Care Support Workers (Band 3) to join our bank and support services across Derbyshire.

You will work within small community support bed and assessment units, providing high-quality, person-centred care to adults with a range of physical, mental health and social care needs. This is a bank / ad-hoc role offering flexible shifts rather than guaranteed full-time hours.

Key responsibilities

  • Provide compassionate, person-centred care in line with individual care plans.
  • Support with personal care, mobility, eating and drinking, and activities of daily living.
  • Complete basic observations (e.g. blood pressure, blood glucose, urinalysis) and escalate concerns appropriately.
  • Promote independence, dignity and wellbeing, including social and emotional support.
  • Work as part of a multi-disciplinary team, following agreed policies, protocols and safeguarding procedures.
  • Maintain accurate records and contribute to safe, effective service delivery.

Shift pattern and sites

  • Services operate 24/7 across:
  • Community Support Bed – Buxton (SK17 7BR)
  • Community Support Bed – Ilkeston (DE7 8PF)
  • Community Support Bed – Staveley (S43 3LY)

Shift times (no long days):

  • 07:00–14:00 (early)
  • 14:00–22:00 (late)
  • 22:00–07:00 (night)

Essential requirements

  • Minimum 1 year of UK experience as a Healthcare Assistant / Support Worker in a health or social care setting.
  • Experience working with adults with physical disabilities, learning disabilities, mental health needs and/or older adults.
  • Ability to travel reliably to at least one of the three sites (Buxton, Ilkeston, Staveley) for early, late and night shifts.
  • Good understanding of safeguarding, confidentiality and professional boundaries.
  • GCSE Maths and English (or equivalent) and one of:
  • NVQ Level 3 in Health & Social Care, or
  • Equivalent health/social care qualification, or
  • Equivalent proven experience in a CQC-regulated organisation with Care Certificate evidence.

Compliance

  • Right to work in the UK (we cannot provide sponsorship for this role).
  • DBS (preferably on the Update Service).
  • Able to provide references covering the last 3 years.
  • Willing/able to provide immunization evidence as required (e.g. TB, Hep B, Varicella, MMR).
