Client Care Coordinator

Job DescriptionWe are looking for a dedicated and compassionate Care Coordinator to join our team, helping to deliver exceptional care to clients in their own homes. This role combines office-based coordination with hands-on support in the communityKey Responsibilities:Conduct service reviews and develop client support plansCreate, update, and audit care plans using digital care planning toolsMaintain accurate records on Home Instead software and People PlannerCarry out client and Care Professional introductions and reviewsSupervise and mentor Care Professionals, supporting their developmentProvide support with care calls in the community when requiredAssist with scheduling and rota management as neededBuild strong relationships with clients, families, and external professionalsEnsure compliance with equality, diversity, and safeguarding policiesSupport the Client Experience team and contribute to continuous service improvementCarry out any other duties as directed by your line manager to ensure smooth business operationsEssential Requirements:Strong understanding of Health & Social Care regulationsFull-time role- Participate in on-call duties, offering out-of-hours support to clients and Care Professionals, weekdays & weekendsExperience delivering personal care and working with MAR sheetsExcellent communication and relationship-building skillsConfidence using care management technology and IT systemsFull driving licence and access to transportPassion for providing outstanding care and supporting independent livingAbility to work flexibly, including evenings and weekends as part of the on-call rotaCommitment to confidentiality and professional standardsQualificationsAdditional InformationIf you’re someone who genuinely cares and wants to make a meaningful impact in people’s lives, we’d love to welcome you to our team at Home Instead Monmouth.Safeguarding is central to everything we do.
