Exciting Opportunity: Advanced Clinical Practitioners – Integrated Specialist Palliative Care Team (2 Posts)

Due to service expansion, we are seeking two motivated and compassionate Advanced Clinical Practitioners (ACPs) to join our Integrated Specialist Palliative Care Team.

This role involves working across inpatient, outpatient, community and virtual ward settings, providing expert assessment, symptom management and care for patients with complex palliative and end‑of‑life needs. ACPs act autonomously, collaborating with consultants and the multidisciplinary team, and serve as clinical leaders and experts.

Opportunities include contributing to service development, quality improvement, audit, research, education and training.

Main duties of the job

  • Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills.
  • Review patients and provide care in inpatient, outpatient, virtual ward and home visit settings.
  • Contribute to a multidisciplinary approach to patient management.
  • Provide education and support to patients, carers and colleagues.
  • Participate in audit, research and service development within the Specialist Palliative Care service.

Job responsibilities

  • Perform independent specialist assessment of patients to formulate a diagnosis, plan and implement care, and arrange ongoing follow‑up as an advanced practitioner using highly developed specialist clinical knowledge and advanced clinical skills.
  • Independently manage a patient caseload providing expert specialist assessment, planning and evaluation.
  • Lead the multidisciplinary team in resolving patient problems by providing new and innovative models of care.
  • Act as a role model and expert clinician, providing in‑depth specialist knowledge to colleagues through MDT meetings, for patients and carers/relatives.
  • Provide leadership, guidance and advice to staff on operational and professional issues, promoting an open, honest and transparent culture.
  • Improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.
  • Provide a consistent and personalised service, ensuring patients receive the best care, in the best place from the best person or team.
  • Offer social, emotional and psychological support for patients from referral and throughout their treatment.
  • Deliver expert specialist evidence‑based care, including development, assessment, planning, implementation and evaluation of programmes of evidence‑based nursing care for a defined caseload.
  • Makes appropriate referrals to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings.
  • Be aware of risk management issues both locally and nationally, and develop strategies to address identified issues.

Person Specification

Qualifications

  • Clinical Professional Qualification – degree level with appropriate registration.
  • MSc Advanced Clinical Practice (ACP) or enrolment in the MSc ACP pathway/programme, committed to completion within an agreed time frame (post holder will be in a training post until evidence of completion of MSc ACP is provided).
  • Teaching and assessing qualification.
  • Prescribing qualification.
  • Evidence of participation in audit and research.

Experience

  • Demonstrable relevant experience within the speciality.
  • Evidence of effective management.
  • Evidence of effective leadership skills.
  • Evidence of staff development.
  • Evidence of change management.
  • Evidence of application of current clinical service issues and developments.

Knowledge

  • Understanding and application of healthcare governance.
  • Has an up‑to‑date Advanced Clinical Practice portfolio.
