OT: Discharge & Home Independence Specialist

Liquid Personnel is seeking an Occupational Therapist for the Integrated Discharge Service in Islington. You will assess risk and design interventions to support individuals discharged from hospital, promoting independence and recovery.

Your responsibilities include post-discharge assessments and co-developing re-ablement plans in collaboration with patients. A degree in Occupational Therapy and HCPC registration are required. The position offers flexibility with home and office-based duties.
