Job overview
This role delivers compassionate care to support people to remain as independent as possible in their own homes, working as part of the Homefirst East Team. The area is rural with no public transport; you must be able to travel independently, often using your own vehicle. Travel costs during work time are supported, and pool cars are available where possible.
You will need excellent communication skills and a positive approach to people who may have communication difficulties or challenging, complex health needs. The post is lone working and requires prior experience in a community setting.
Duty hours: 08:00 to 20:00, 7 days a week including bank holidays, with flexibility within the rota and geographical location.
Main duties
- Provide a versatile and responsive service to optimise opportunities for patients’ re‑ablement within the community setting.
- Assist patients to achieve their re‑ablement goals using a range of specific activities, supporting independence in daily living skills and personal care, and promoting a return to normal function and activity with appropriate aids and equipment.
- Be part of an Integrated Care Pathway for older patients that involves a range of services to prevent hospital admission and support discharge from hospital.
- Deliver compassionate care to patients in their own home.
- Report concerns or issues to the manager in charge and maintain accurate records using various IT systems.
- Make or receive telephone calls and communicate effectively with other professionals, patients and their relatives as required.
Person specification
Education, qualifications and experience
- NVQ Level 3 qualification in care or equivalent experience.
- Evidence of experience in a care setting.
- Additional relevant training in healthcare.
Skills and aptitude
- Flexible in attitude and willing to support in other teams when required.
- Ability to plan own workload and organise required activities.
