Overview
We have an exciting opportunity for a Band 3 Rehabilitation Assistant who is dynamic and passionate about stroke rehabilitation to join our supportive and caring team.
The Community Stroke Rehabilitation Team offers specialist multi-disciplinary rehabilitation to adults who have had a stroke in a community setting. You will work closely with other professionals such as occupational therapists, physiotherapists, speech and language therapists, psychologists, dietitians, nurses and therapy assistant practitioners. The service provides goal focused, time limited rehabilitation interventions in patient’s home environments and in other settings as appropriate.
The stroke rehab team works closely alongside colleagues from Leeds Teaching Hospital Trust, as well as third sector organisations such as the Stroke Association, to support patients along the whole stroke pathway, from hospital to home.
This is an exciting time to join the team, as we embark on implementing a new intensive rehabilitation pathway to support earlier discharge from hospital alongside our current offer.
Key Responsibilities
Carry out care plans created by occupational therapists, physiotherapists, speech and language therapists and dietitians. Work jointly with members of the MDT when two staff members are required. A full competency and training framework is in place.
Some weekend working is required.

Qualifications & Experience
The ideal candidate will have experience of working with people who have had a stroke or other neurological conditions, working in an MDT setting and community working.
