Overview
Working within a Reablement model of care, the role involves providing advice and guidance to Support Workers within the locality and case managing more complex cases.
Responsibilities
- Provide advice and guidance to Support Workers within the locality.
- Case manage more complex cases.
- Cover shifts on an ad hoc basis, including evenings, weekends and bank holidays between 8am and 8pm.
Essential Requirements
- Valid driving licence and use of own car/vehicle.
