Position:
Essence Dementia Navigator (Phase 1)Permanent contract, 35 hours per week – £25,935 per annum.
Responsibilities
- Provide one‑to‑one support for people with mild to moderate dementia and their carers via tailored personal plans.
Requirements
- Full clean driving licence and access to a car for work.
- DBS Enhanced Disclosure check.
- Excellent verbal and written communication abilities.
- IT skills and experience/knowledge of working with people with dementia.
