Band 3 Community Support Worker – Community Response Team
We are looking for an enthusiastic and motivated Band 3 Community Support Worker to join our Community Response Team covering Rugby and surrounding areas.
About The Role
The Community Response Team supports patients who have identified health and social care needs following a change in their condition. The service is committed to delivering high‑quality, responsive care, with an immediate service aimed at:
- Preventing avoidable hospital admissions
- Supporting patients to remain independent in their own homes
- Facilitating safe and timely discharges from hospital
You will be part of a multidisciplinary team, working collaboratively to support patients in the community. This includes close partnership with:
- Social services
- General practitioners (GPs)
- Community nursing teams
- Patients, families, and carers
The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction but not direct supervision of registered staff, working as part of the multidisciplinary team.
