Are you a dynamic, confident and motivated Mental Health Nurse, Occupational Therapist or Social Worker with excellent communication skills and the ability to assess and lead on clinical decision making? If so, we would like to hear from you. This exciting role works in a fast‑paced, multi‑disciplinary team delivering the Crisis and Home Treatment Team service, providing high‑quality assessment and home treatment as an alternative to hospital admission for individuals experiencing acute mental health crisis.
We require you to obtain prior approval from your current line manager before submitting your application for this secondment opportunity.
Main duties of the job
- Provide first line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
- Rapidly assess people with mental health problems who are vulnerable to hospital admission.
- Provide specialist crisis assessments with a focus on “hospital at home”.
- Coordinate the Crisis and Home Treatment Team on a shift‑by‑shift basis, acting as Duty Worker to triage referrals, plan shift activities and team the service accordingly.
- Participate in team MDMs.
Qualifications
- Qualified Mental Health Nurse, Occupational Therapist or Social Worker.
- Current registration with the relevant professional body: NMC, HCPC, Social Work England.
Experience
- Previous and relevant post‑registration experience.

- Ability to meet the travel requirements of the post.
