What you will do:

• Conduct comprehensive assessments of individuals’ social care needs within the community
• Develop, implement, and review personalized care plans in collaboration with other health professionals
• Provide expert guidance and support to service users and their families
• Work closely with multidisciplinary teams to ensure coordinated care
• Maintain accurate records and produce reports for case reviews
• Stay updated on legislation and social work best practices to ensure compliance

Requirements:

• Degree or equivalent in social work
• Current registration with Social Work England
• Substantial experience in frontline social services within the UK as a qualified Social Worker
