Overview
This is an exciting time to join our Older People’s Mental Health Team (OPMHT) based in Quarella Road in Bridgend. The OPMHT is a multi-disciplinary team that provides services to individuals who are in receipt of Secondary Care as defined by the Mental Health (Wales) Measure 2010. As a social worker within the team, you will work jointly and collaboratively alongside health colleagues. The team is co‑located with Community Mental Health Nurses, Mental Health Occupational Therapists and Physiotherapists. We also work closely with Angelton Clinic, located on the Glanrhyd Hospital site in Bridgend, which provides an inpatient service for older people with serious and enduring mental illness and dementia. We collaborate with Consultant Psychiatrists and their team to support individuals in both community and hospital settings.
Responsibilities
- Provide a strengths‑based, outcome‑focused and enabling response to adults aged 65 and over affected by mental health conditions.
- Undertake complex case work with individuals and their carers.
- Conduct holistic assessments and develop person‑centred Care and Treatment Plans.
- Act as an allocated Care Coordinator for individuals in receipt of secondary mental health services, as detailed within Part 2 of the Mental Health (Wales) Measure 2010.
- Co‑produce outcome‑focused Care and Treatment Plans, undertaking risk assessments and promoting individuals’ rights.
- Promote independence and well‑being for individuals, their families and carers, focusing on what matters to people.
- Focus on prevention and self‑management where practicable, enabling individuals to sustain independence through assessment, care and support and other practical interventions.
- Ensure choice and control for individuals, placing the person at the centre of their care and ensuring their voice and choice are heard.
Qualifications and Requirements
- Social work background with experience in community or mental health settings.
- Ability to greet customers through the medium of Welsh.
- Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS).
- A valid driving licence.
- Commitment to the council’s Hybrid Working Policy.
- Strong team working and interpersonal skills.
