We are seeking an experienced and dedicated Senior Social Worker to join our dynamic team – Caradon CMHT within the Cornwall Partnership NHS Foundation Trust Mental Health Service. Main Duties of the Job

This is a community‑based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

  • Toberesponsiblefortheassessmentofcareneedsandclinicalmanagementofadefinedcaseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
  • Tocarrycontinuingresponsibilityforthedevelopment,implementationandevaluationofprogrammes of care.
  • Todeveloptheinterfacebetweenprimaryandsecondarycareservicessothepatients/carersare offered focused and co‑ordinated packages of care that are appropriate to their needs.

Person Specification

Education and Qualifications

  • Relevant professional registration – Social Worker
  • Evidence of post basic training
  • Current professional registration

Experience

  • Extensive experience of working with mental health patients
