Job Overview

Ceredigion County Council’s Targeted Care & Enablement Team is recruiting support workers to assist individuals in regaining skills and independence following acute illness or injury. The role involves daily living activities such as personal care, mobility support, meal preparation and medication assistance in clients’ own homes.

Key Responsibilities

  • Provide personal care, mobility support, meal preparation, and medication assistance to clients in their own homes.
  • Support clients to achieve OT‑led, client‑centred goals for independence.
  • Cover long‑term bridging duties while awaiting package sourcing.
  • Manage daily schedule using an electronic call monitoring system, ensuring punctuality and adherence to prescribed times.
  • Travel within Ceredigion to client homes using your own vehicle; full driving licence required.

Working Hours

Split shifts on a 4‑days on / 4‑days off rota. Daily hours: 7-1 and 4-8.

Qualifications & Requirements

  • Full driving licence and own car with ability to travel within Ceredigion.
  • Level 2 QCF (or equivalent) in health and social care.
  • Basic conversational Welsh; verbal level 3 (or level to be achieved within a reasonable timescale). Reasonable support will be offered.
  • Completion of All Wales Induction Framework (AWIF) for Health and Social Care within the first six months to register as a Social Care Worker with Social Care Wales.
  • Enhanced DBS check required; prior convictions will be considered on a case‑by‑case basis.
