What you will do

• Conduct assessments and create care plans to support the individual needs of children and young people with disabilities.
• Work closely with families, carers, and other professionals to ensure a holistic approach to care.
• Provide advice, guidance, and support to families and young people, helping to navigate complex situations.
• Keep accurate records and reports, ensuring that information is shared appropriately with relevant parties.
• Regularly review care plans and interventions to ensure they continue to meet the needs of young people.
• Advocate for the rights and wellbeing of children and young people with disabilities, ensuring their voices are heard.
