Transition Social Worker for 16–18 Leaving Care

Connect2Halton is seeking an experienced Children’s Social Worker to join the Children in Care Team. You will support young people aged 16 to 18 as they transition to independence, part of a dedicated team achieving positive outcomes for looked after children.

Responsibilities include managing a caseload, conducting statutory visits and care planning, and collaborating with various professionals. Qualified Social Worker status and at least two years experience in children’s social care are essential.
