Derbyshire Dales North Adult Social Work Team are based in Matlock, and we are seeking to recruit two full time 37 hours per week experienced Social Workers to join our friendly, community-based team of Social Care Practitioners and Social Workers.
The team is highly supportive and operates on a largely agile, flexible basis across Derbyshire Dales North area of Derbyshire, utilising link worker and champion roles to carry out Care Act (2014) face‑to‑face and telephone assessments and reviews.
Applicants must evidence high levels of autonomy and accountability with timely recording of interventions to progress overall case management. They must demonstrate the skills to build positive, professional links with others to work closely with our partner agencies, such as Health, Police, Housing and PVI sectors. The ability to identify signposting opportunities is necessary to support carers, maximise independence and individual wellbeing with a focus on promoting person‑centred, strength‑based outcomes.
The role sometimes requires duty cover; applicants must be able to respond to changing situations, triage and assess incoming work to support Adults and their carers who may require a more urgent response. It is vital to be able to evidence experience and skills to manage a complex caseload whilst meeting statutory duties in a safe, accountable, and cost‑effective manner to meet Care Act (2014) eligible needs alongside local authority statutory duties of care.
It is an advantage to have experience of taking the lead on Section 42 Safeguarding, chairing multi‑agency meetings such as Multi‑Agency Adult Risk Management (MARM) and completing Mental Capacity Act (2005) assessments, including Best Interest meetings, COP‑DOLs and court work; support, training and experience will be available for the right candidate to develop skills in these areas.
You will work Monday to Friday and have a minimum of monthly 1:1 supervision and plenty of opportunities for informal support with both your team, peers, and line manager.
