About the Role

An exciting opportunity has arisen for an experienced Adult Social Worker to join Wolverhampton’s Ageing Well Team. This rewarding role offers the chance to support older adults to maintain their independence, enhance their wellbeing, and access the care and support they need to live safely within their communities.

Working within a collaborative multidisciplinary team, you’ll deliver person‑centred, strengths‑based social work that makes a meaningful difference to the lives of older people and their carers.

Key Responsibilities

• Manage a caseload of older adults with a range of physical, mental health, and social care needs.
• Undertake Care Act 2014 assessments, reviews, and support planning.
• Complete safeguarding enquiries and risk assessments where appropriate.
• Work closely with individuals, families, carers, health professionals, and partner agencies to achieve positive outcomes.
• Promote independence through strengths‑based practice and person‑centred care planning.
• Support hospital discharge planning and community‑based interventions where required.
• Maintain accurate and timely case records in line with statutory requirements and professional standards.
