Overview of the Learning Disability Team/Service
The Learning Disability Team provides specialist support to adults with learning disabilities, ensuring that they receive the right care, support, accommodation, education, and health services to achieve positive outcomes. The service works collaboratively with individuals, families, carers, health professionals, education providers, and community organisations to promote independence, wellbeing, safety, and inclusion.
Impact of the Role on the Organisation
The role within the Learning Disability Team has a significant impact on both the organisation and the individuals it serves. It contributes to:
- Improved outcomes for individuals and families by ensuring timely assessments, interventions, and support arrangements.
- Safeguarding and risk management, protecting vulnerable individuals and ensuring statutory responsibilities are met.
- Efficient use of resources through effective commissioning, care planning, and placement management.
- Service improvement and quality assurance by monitoring performance and driving best practice.
- Partnership working, strengthening relationships with health, education, housing, and community services.
- Reduced placement breakdowns and crisis interventions through proactive support and robust planning.

- Compliance with statutory duties, ensuring the organisation meets its legal obligations under relevant social care and equality legislation.
Working Hours
This is a part-time opportunity, working 30 hours per week.

What will you be doing?
Key Responsibilities
Complete strengths-based assessments to identify care and support needs. Develop and review person-centred support plans.
Undertake safeguarding enquiries to protect adults at risk.
Carry out Mental Capacity Assessments and Best Interest decisions. Assess and manage risks while promoting individual rights and wellbeing.
Support individuals and families through complex transitions. Work closely with health professionals and partner agencies.
Coordinate multi-agency meetings and collaborative working. Prepare reports/checklists for Quality Assurance Forum (panels), courts, and safeguarding processes. Monitor and review services to ensure positive outcomes.
Maintain accurate and up-to-date case records. Complete Community Deprivation of Liberty applications to the Court of Protection and accompanying assessments.
About you
These are the relevant skills, qualifications and required experience for the job:
A professional qualification in social work – with a Social Work England registration. Good knowledge of The Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and safeguarding legislation. Strong assessment, analytical, and decision-making skills.
Experience supporting adults with learning disabilities and complex needs. Ability to manage a demanding and complex caseload. Excellent communication and relationship-building skills.
Experience of multidisciplinary and partnership working such as working collaboratively with health colleagues, including Community Learning Disability Nurses, Psychologists, Occupational Therapists, and other professionals within the Partnership Team.
Strong report-writing and record-keeping abilities. Commitment to strengths-based, person-centred practice. Completing carers assessments and supporting carers in their caring role.
