- 1 x Social Worker – Working Age Adults Mental Health Team
- 1 x Social Worker – Older Person’s Mental Health Team
- 1 x Social Worker – Complex Care Team
- 2 x Social Workers – Transition’s Team
Responsibilities
In this key role, you will work with adults with a range of complex care and support needs, ensuring their safety, wellbeing and independence through effective, strengths‑based practice. You will undertake proportionate and person‑centred assessments, manage a complex caseload, and develop outcome‑focused care and support plans. Working collaboratively with multidisciplinary colleagues and partner agencies, you will coordinate interventions that reduce risk, promote independence and support individuals to achieve their goals.
The role requires confident professional judgement in complex situations, including safeguarding activity, and the ability to balance risk, choice and control in line with statutory responsibilities. You will also contribute to a culture of high‑quality practice through effective communication, clear recording, and strong analytical report writing.
Essential Skills
- Professional judgement within statutory frameworks with clear decision‑making rationale
- Application of Adult Social Care legislation, proportionality and least restrictive practice
- Role model for good practice within multidisciplinary teams and reflective practice
- Provision of information and advice on hospital discharge planning, funding and commissioned services
- Lead role in jointly worked complex cases
Essential Knowledge
- Safeguarding Personal and strengths‑based social work practice
- Professional standards and regulatory requirements relevant to Social Work England
- Advanced knowledge of the Care Act
- Proven knowledge of the Mental Capacity Act
- Managing expectations
Essential Experience & Qualifications
- Relevant professional Social Work qualification – Degree/Diploma in Social Work, Certificate of Qualification in Social Work, or Certificate in Social Services.
- Registered with the professional body Social Work England.
- Commitment to continued professional development.
