Social Worker – Rapid Response and Reablement Team

The States of Guernsey are looking for a Social Worker to join the Rapid Response and Reablement Team.

Main duties of the job

The primary purpose of the post is to deliver a high-quality, evidence-basedcasework service to all service users, holding cases within the Rapid Response and Reablement caseload.

The post holder will be based in the Integrated Community Health and Social Care Service, attached to the Rapid Response and Reablement Team. The role will involve providing consultation, support, specialist advice and joint working across integrated services to ensure the delivery of person-centred care and support.

The post holder will take a leadership role within the team, acting in an advisory capacity to members of the multidisciplinary team on adult social care matters, and providing education and mentoring where appropriate. In addition, the post holder will contribute to the development of practice within the service by delivering training, consultation and supervision to less experienced social workers, support workers and student social workers within this area of practice.

Job responsibilities

Responsibility for Patient/Service User Care

To promote and deliver a high standard of care based on best practice to maintain patient/service user safety and provide an excellent.

To coordinate the assessment of patients/service users with complex needs including individuals with unconfirmed diagnosis.

To coordinate the development, implementation and evaluation of specialised programmes of care utilising evidence and research.

To lead a team of staff/provide specialist advice in acute and chronic situations ensuring that the patients/services users needs are met along with their support network.

To ensure patient/service user involvement in the planning, delivery and evaluation of care, respecting their beliefs and dignity.

To utilise data and information from a range of sources to identify patient/service user condition to guide decision making.

Within field of expertise and policy and where appropriate, request specific clinical tests to establish a diagnosis.

To undertake the role of health promoter and educator for patients/service users and their support networks.

To ensure that professional standards are upheld by the staff members working with the postholder’s sphere of responsibility.

To manage all incidents as per policy and participate in risk management and Quality and Safety processes e.g SWARM.

To be accountable for ensuring that safeguards are in place for patients/service users and escape any safety, safeguarding or risk concerns following the appropriate processes.

Communication

To build and advocate a therapeutic relationship with patients/service users and their support network, communicating with them in a way that respects their views, language, cultural differences and cognitive function.

To use and promote a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations.

To manage conflict between individuals including staff, patients/service users and their support network.

To establish and promote effective working relations with all members of the multidisciplinary team and outside agencies, clearly relaying condition related information in a timely and effective manner.

Communicates to patients/service users and support networks compassionately, ina confidential and sensitive professional manner.

To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.

Leadership

To act as a professional role model for staff, advocating for patients and service users and promoting high standards of practice and strong professional values.

To lead the department or service.

To lead and coordinate nurse led and multi-agency activities within area of specialist practice.

To lead on the investigation of complaints and clinical incidents related to own clinical area, ensuring that learning is shared with own team and wider HSC.

To promote own and team success and excellent standards of care by reflecting, challenging others constructively, upholding HSC’s CARE Values.

To ensure team members are aware of organisational objectives, policies and procedures and implications that they have for their practice.

To undertake Values Based Appraisals for staff as required and support such staff with performance issues, developing action plans to address developmental needs.

To lead in the recruitment of team members and ensure a positive and supportive working environment for all staff.

To advocate on behalf of patients and service users.

To take accountability and reflect on own decisions and actions using available support networks.

Planning and Organisation

To organise own workload and that of others, demonstrating the ability to recognise changing priorities and adjusting plans accordingly.

To lead with shift planning/work scheduling for the service area including the appropriate allocation of staff to ensure safe levels of care.

To lead, coordinate and delegate activities to colleagues where applicable within agreed systems of working, considering required skill mix and staffing levels, in accordance with patient/service user need.

Policy and Service Development

To lead and contribute to service developments and propose changes to policies and procedures, ensuring that new initiatives are incorporated into working practices of staff.

To support the senior management team to initiate and implement improvements using a systematic approach using KPIs and clinical incidents.

To ensure that staff members are working with the scope of relevant policies, procedures and guidelines.

Partnership Working

To demonstrate an understanding of and apply the principles of the CARE Values.

To actively work and promote collaborative working as part of a multi-disciplinary team with a range of professionals from other service areas who may be participating in the care of a patient/service user.

To lead effective team working, communicating with others in a constructive way, which enables colleagues to make suggestions for the improvement of care delivery.

Through effective negotiation and organisational and political awareness, create an environment for partnership working and dissemination of information across multi disciplinary teams.

To lead and participate in team meetings.

Responsibility for Physical and Financial Resources

To ensure that public funds are used responsibly and appropriately.

To authorise expenditure, e.g. the signing of timesheets and authorising the use of additional staff.

To effectively manage resources within the service area e.g equipment and supplies.

To manage expenditure within budget for service area and report to the senior manager, any actual or potential deviations from the budget.

To ensure all staff handle valuables in accordance with agreed policies and procedures.

Professional Development

To take responsibility for maintaining own, and that of others within the service, professional development and revalidation requirements, ensuring completion of training in accordance with the Statutory, Mandatory and Essential to Role Training Policy.

To participate in and lead supervisory one-to-one meetings and HSC’s Values Based Appraisal process, developing plans to meet personal and organisational objectives.

To continually develop new knowledge and skills, applying this to practice and sharing with colleagues as appropriate.

To participate in and develop plans for the induction, orientation and development of new staff, agency workers, students and any voluntary workers.

To lead and coordinate the use of data from clinical incidents and other sources to inform areas of audit activity and service development.

To lead HSC quality improvement initiatives and audits to improve patient and service user safety and care.

To keep up to date and inform others of changes to evidence-based practice and current trends.

Analysis and Data Management

To develop Key Performance Indicators and monitor trends within own service area and lead on service development.

To utilise a variety of data sources to contribute to the analysis and investigation of incidents.

Person Specification

Qualifications

  • Registered Social Worker with Social Work England.
  • Possess a full and valid driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for individuals in line with the Prevention of Discrimination (Guernsey) Ordinance, 2022.
  • Full Qualifications to be achieved by second gateway:
  • Management Qualification Level 5 and above.
  • Key Criteria:
  • The ability to evidence application of HSCs CARE Values in practice.
  • Evidence of leadership and change management with an understanding of quality improvement methodology.
  • Evidence of managing own workload with excellent time management skills.
  • Evidence of the ability to effectively manage resources.
