Social Worker, Transitions to Adult Services (Grade 5)

Eden Brown Synergy is recruiting a Social Worker for the Transitions Team in Great Barr. This role involves supporting young individuals transitioning to adult services by performing essential Care Act assessments and collaborating with families and partner agencies.

The ideal candidate will be a qualified Social Worker with SWE registration and experience in adult social care transitions. The position offers a rate of £28.40 per hour and requires office attendance every Tuesday.
