We have an exciting vacancy for a Social Worker in the Leaving Care Team at Scott House, Huntingdon.

Responsibilities include:

  • Providing skilled, relationship‑based support to care‑experienced young people aged 16–18 as they prepare for adulthood.
  • Leading the development and review of high‑quality Pathway Plans based on thorough assessments, clear goals and the young person’s aspirations, needs, strengths and independence goals.
  • Working collaboratively with schools, colleges, health professionals, housing services and other agencies to create a consistent and joined‑up approach.
  • Using professional judgement to assess and manage risk, respond to emerging needs and advocate for young people across key transitions.
  • Maintaining consistent, trusting relationships and offering practical, emotional and social skills support for successful transition to adulthood.
  • Providing advice, guidance and immediate support as part of the team duty system.

Person specification

  • Qualified Social Worker with Social Work England registration.
  • Ability to work from a variety of locations and travel county‑wide, and out of county, independently.
  • Profile of a confident, dynamic professional with an interest in improving outcomes for young people, trauma‑informed and strengths‑based practice.
  • Capacity to work within a multidisciplinary team and maintain CPD.
